Coach/Player Ejection Report

INSTRUCTIONS: This form it to be completed by the OFFICIAL(S) ejecting a player or coach. The form must be submitted, postmarked or faxed  to the Booking Agent within 48 hours or 2 working days following the completion of the game.  For a participant to be ejected, he/she must commit one of the six acts listed below under Reason for Ejection.  It is not necessary to submit an ejection report for a disqualification.  Please fill out the report completely, filling in all of the blanks.  After filling out the form, print it, and fax it to the appropriate booking supervisor.
